Application Formats & Certificates

Below are the application forms (as thumbnails) that may be downloaded, filled and submitted in the office during working hours. Please note that no other formats will be entertained.

1. Application for Certificates
Application for Certificates Fill the details and tick the required certificate and submit in the office along with the fees. For Instructions click here
2. Application for Change of Address
Application for change of address Submit the filled in application in the office.
3. Application for Duplicate ID/Library Card
Application for duplicate cards Fill the details and take the signature of the concerned and submit in the office along with the fees.
4. Application for Change of Marks
Application for updation of marks Fill the details and submit the same after getting signature from the concerned teacher on or before the given date and time. If students do not correct the mistakes, if any, it may remain as a wrong data entry and will be printed in the Progress report and Cumulative record.
5. Application for Duplicate Hand Book
A letter (A4 size) addressed to the Principal with the details and reason for the loss, countersigned by the Class Teacher and Class Counsellor along with the prescribed fine must be duly submitted in the office.
6. Application for Original Marks Card (Class X - Applicable for I PUC Students Only)
A letter (A4 size) addressed to the Principal containing the details, reason and the date of return of marks card must be duly submitted in the office.
PROCEDURE FOR STUDENT RECORD VERIFICATION

To get a student’s record verified, the applicant must submit

  • a letter addressed to the Principal with all requisite details that will be required to verify the document
  • a copy of the document to be verified
  • a demand draft of Rs. 300/- in favour of the Principal, Christ Junior College, Bangalore
  • The letter addressed to the Principal must clearly explain the purpose for which the verification is being sought.
  • The Office will verify the documents and fax the verified document with the verification result.
  • Requests for online verifications will not be entertained.

Procedure for issuing Transfer Certificate

All the passing out students will receive the Transfer Certificate along with the Original Marks card of II PUC issued by the Department of Pre University Education. The request could be for an original one, that is, one that is issued for the first time or a duplicate one, that is, when the original copy has been lost or misplaced.

Failed students who have collected their 'Failed' TC can request for a Passed TC after they have passed their supplementary exam.

 

Note regarding Certificates Issued by PU Board

II PUC Duplicate Marks Card

Students should fill in the application provided by the Department of Pre-University Education and obtain an affidavit from a notary. They must also carry a copy of an FIR filed with the police. Students should pay the required fee in the Treasury branch, State Bank of Mysore, Shivajinagar (Opposite to Bowring Hospital). The application, affidavit and fee paid challan should be submitted to the Department of Pre-University Education by the student.

 

Migration Certificate

Students, who wish to continue their higher studies outside Karnataka need to apply for Migration Certificate. They can approach the Department of Pre-University Education along with II PUC Original Marks Card and Transfer Certificate Applications are available in the dept. of PUE and the college office.

 

II PUC With draw of Result (subject)

Students must submit the filled in application form, Affidavit, Original Marks Card (Passed/Failed) and prescribed fees at the PU board Karnataka and obtain permission from, after which the examination fee may be paid at the college office.

 

Photocopy of Answer Books, Revaluation and Re-totalling of Marks

Students can apply for Photocopy of Answer Books, Revaluation and Re-totalling of Marks of II PUC Examination at the Department of Pre-University Education with prescribed fee immediately after the II PU results. Applications are available in the college office.

  • Submit the filled in application form along with a request letter in the office.
10.
Date of Birth Certificate
  • Submit the application with a request letter and photocopy of the X std. Marks card in the office.
11.
I PU Duplicate Marks Card
  • Submit the application to the Principal along with a request letter, affidavit from a notary and Acknowledgement of FIR filed with the police
12.
Duplicate Fee Challan
  • Submit the application with a request letter to the Principal.
13.
Visa Letter
  • Submit the application with a request letter and photocopy of passport to the Principal.
14.
Reference letter from Teachers
  • Meet the concerned teacher with the request letter and submit along with application and soft copy of the letter.
